Contact your beat constable or the nearest police station for the verification form. If you live in Delhi, Mumbai or Kolkata, you can download forms from your city police website. Some of the key information required is as follows:
Photographs of the candidate
Complete local and permanent address
Some identification (ration card/
Driving license/voter’s ID card)
Name and address of the village sarpanch
• Addresses of relatives, friends or other acquaintances in the city
• Personal identification marks
• Signature
• Handwriting specimen
• Fingerprints for both hands
Once you have submitted the form, the police will check all their addresses, contact previous employers and match fingerprints with their own database to ensure that the person has no criminal records.
